We will enrol 10-20 people per week. If the waiting time becomes more than SIX weeks, we will pause it while we catch up.
**If the form below is NOT visible, then it is paused. Just keep checking once a week, till it reappears. We intend to be enrolling regularly at this rate all year. **

We will ONLY take waiting list requests via the form below, and will process in the order received. We will try to indicate weeks you will be waiting, and if you wish to withdraw your application please let us know. Please do not phone or email us to find out your place on the list.
ENROLMENT requirements:
- We will only enrol people who are NOT currently enrolled at a medical centre within 15km of Rolleston Medical Centre. Our intention is to provide healthcare to those without a local GP already. You cannot currently be enrolled at Rolleston Central Health, We Care Lincoln/We Care Wigram/ We Care Prebbleton, Lincoln Medical, Phoenix Health, or West Melton/Darfield Medical.
- Our designated enrolment zone is the Rolleston township and some close surrounds. We are currently including WestMelton Township, Dunsandel township, Weedons, and some rural areas in between.(Map above) . You must live in this zone. The manager will use their discretion if you’re just on the outer of this zone.
- We will not be accepting Lincoln, Leeston, Springston, Kirwee, Darfield or Templeton residents- you have good access to other nearby centres.

Rolleston Medical Centre is a member of Pegasus PHO (Primary Health Organisation).
Benefits of enrolling with a PHO practice:
- You will be eligible for reduced family doctor fees including ACC visits
- Reduced prescription charges
- Free sexual health visits (Under 21)
- Free Palliative Care visits
- Acute Demand Care – GP and Nurse-led care to prevent hospital admission through monitoring in the community
- Monitoring and screening for national recalls (Cervical Cytology, Mammography)
- Free childhood immunisation
Patients 16 years and older are required to sign and date their own forms.
Children under 16 years, will have their forms signed and dated by a parent or guardian.
You can enrol at only one Medical Practice.
